Pinnacle Health Management (PHM) is a health-based care management company that offers employers a variety of employee benefit solutions. Our Health Management program offers a team of supportive health professionals, who work closely with employers, to assist employees in managing persistent and complex health condition(s), improving health outcomes and preventing related risks and complications.
Pinnacle Health Management programs take a systematic approach to helping patients diagnosed with persistent and complex health conditions. Our clinical staff works directly with providers and independent review organizations to address and resolve gaps in care, such as medicine adherence and assistance with specialty referrals. Our curriculum approach to health education includes one-on-one calls with a care manager to provide members with the tools and support to inspire lifestyle changes. We’re constantly adding new programs, including wellness initiatives, to effectively meet the needs of clients and their employees.
Pinnacle Health Management is independently owned and has more than 20 years of experience in the health benefits industry. Our team consists of registered nurses, care coordinators, and licensed behavioral therapists, all of whom support and advocate wellness, prevention of uncontrolled disease states, and the provision of the highest quality of care.
Pinnacle is headquartered in Irvine, California and operates several locations throughout California and Arizona. Pinnacle is SSAE 18 (SOC1) Type II certified and HiTrust (SOC ll) Type 1 compliant with designations issued by outside auditors that verify the correct policies are in place and being followed.
