How It Works

  1. A care coordinator will enroll you via phone and assign a care manager as your coach throughout the program
  2. An introductory call with your care manager will help you identify and set collaborative health goals
  3. You will receive helpful educational resources that will be used during your check in calls with your care manager
  4. Your care manager is available for questions or concerns in-between check in calls, as well as to help you coordinate any specialty referrals.
